Requirements:

• Registered Social Worker with 3+ years in palliative fields

• Familiarity with community care resources

• Strong partnership-building and communication skills

• Driver’s license and vehicle for home visits essential

• Mandatory Police Vulnerable Sector Check and vaccination proof
