You'll provide psychosocial evaluations, counseling, and crisis intervention to ensure effective management of mental health and addiction challenges faced by seniors. Reporting to the Manager of Caregiver Wellness, your expertise will help navigate clients through complex emotional and cognitive landscapes while fostering community connections to essential resources.
Key Responsibilities:
• Conduct geriatric and psychosocial assessments thoroughly
• Implement tailored service and care plans effectively
• Offer counseling support and crisis management
• Advocate for clients' needs with caregivers and families
• Lead engaging group therapy sessions as needed
Requirements:
• Master's Degree in Social Work (M.S.W.)
• Registered as a Social Worker in Ontario
• 3+ years in geriatrics, mental health, or addictions
• Strong skills in therapeutic approaches and advocacy
• Knowledgeable in Alzheimer’s and dementia care
