Flemingdon Health Centre is seeking a dedicated Community Health Worker for its locally driven Population Health Model Program. This full-time position focuses on coordinating with Community Health Ambassadors and partner organizations to support health promotion and education. You will be instrumental in building relationships and enhancing community engagement through effective communication and collaboration.

Key Responsibilities:

• Coordinate logistics between FHC and partner organizations

• Support Steering Committee and partner meetings

• Train and engage Community Health Ambassadors (CHAs)

• Track budgets and program evaluation activities

• Maintain program files and support funding applications

Requirements:

• Bachelor’s degree in health or social sciences

• Experience with equity-deserving communities

• Knowledge of systemic barriers in health access

• Proven community outreach and engagement skills

• Ability to work independently and in teams
