Your role will encompass a range of responsibilities, including conducting psycho-social assessments, implementing therapy sessions, and crafting discharge plans in coordination with the treatment team. Ideal candidates will bring a master’s degree in social work and two years of pertinent experience to effectively serve the North West Toronto community.
Key Responsibilities:
• Complete addiction assessments for patients referred
• Conduct therapy sessions adopting various modalities
• Collaborate on discharge planning with the healthcare team
• Document patient information and progress meticulously
• Engage in crisis intervention and support
Requirements:
• Master of Social Work from an accredited school
• Active memberships with relevant social work organizations
• Two years' experience in mental health/addiction settings preferred
• Knowledge of assessments and treatment models
• Excellent communication and decision-making abilities
