Program Coordinator – HIV/Hepatitis C

In this crucial role, you will implement prison in-reach programs and case management tailored for individuals living with or at risk for Hepatitis
C. Your strong background in harm reduction, social determinants of health, and community support will be pivotal in improving access to testing and treatment. Collaborating with diverse community stakeholders will enhance our service delivery efforts.
Key Responsibilities:
• Manage implementation of prevention and support strategies
• Foster partnerships within the health and social services sector
• Support reintegration efforts for individuals with Hep C
• Deliver capacity-building training for service providers
• Maintain detailed records and evaluation metrics
Requirements:
• Deep knowledge of HIV and Hep C issues
• Exceptional organizational and communication skills
• Experience in program monitoring and evaluation
• Ability to travel and work independently
• Vulnerable Sector Check clearance for provincial prisons
