Community Health Worker at Flemingdon Health Centre

Drive health education and promotion as a Community Health Worker at Flemingdon Health Centre. Support equity-deserving communities through coordination, partnerships, and health programming.

This full-time role focuses on implementing the Community Health Ambassador Model. You will coordinate with East Toronto and North York Health Partners, build relationships with community organizations, and support Community Health Ambassadors. Your work will enhance health access and provide essential community engagement while adhering to a strong commitment to equity.

Key Responsibilities:

• Coordinate logistics between FHC and partner organizations

• Build strong relationships with community agencies and schools

• Support CHA training, collaboration, and ongoing engagement

• Track and evaluate CHA program budgets and schedules

• Administer funding applications and maintain program records

Requirements:

• Bachelor’s degree in health or social sciences

• Experience with diverse, equity-deserving communities

• Knowledge of systemic barriers in health access

• Demonstrated community outreach and relationship-building skills

• Ability to manage competing priorities and deadlines
