Advanced Practice Aftercare Counsellor

You will deliver evidence-informed aftercare groups while ensuring clients have access to necessary community services. This full-time, 18-month contract requires your experience in mental health and substance use, focusing on women’s individual needs. Engage in monitoring recovery progress and reducing client isolation through meaningful follow-up interactions.
Key Responsibilities:
• Launch and facilitate targeted aftercare groups for clients
• Monitor client recovery progress through check-ins
• Connect clients with local resources and services
• Collaborate with teams to support integrated client care
• Document interactions and prepare reports when needed
Requirements:
• Master’s degree in counseling or related field
• Minimum 3 years in therapeutic environments
• Proven experience in group facilitation
• Strong understanding of community resources
• Ability to work collaboratively in diverse teams
