Background:

Flemingdon Health Centre (FHC) is a registered charity and not-for-profit Community Health Centre serving Flemingdon Park, Fairview, and Thorncliffe Park. Across three sites, FHC provides health-related services grounded in the social determinants of health, community engagement, equity, inclusion, accountability, excellence, collaboration, and partnerships.

Position Summary:

The Community Health Worker supports the implementation and coordination of the Community Health Ambassador Model across Flemingdon Health Centre’s sites. As lead agency, FHC works in coordination with East Toronto Health Partners, North York Toronto Health Partners, partner agencies, a Steering Committee, and Community Health Ambassadors. This role supports equity-deserving communities in FHC’s catchment area by advancing health education, health promotion, and system navigation.

The Community Health Worker works closely with CHAs at FHC and supports coordination of CHA activities across partnership sites.

Responsibilities include:

Coordination and Partnerships
  • Coordinate logistics between FHC and partner organizations.
  • Support Steering Committee and partner meetings to strengthen coordination and communication.
  • Build relationships with community agencies, coalitions, schools, and key stakeholders.
CHA Coordination, Training, and Team Support
  • Provide day-to-day coordination and practical support to CHAs.
  • Support CHA onboarding, learning, peer collaboration, role clarity, and ongoing engagement.
  • Organize training opportunities that strengthen CHA skills, knowledge, confidence, and employment pathways.
Planning, Reporting, and Evaluation
  • Track budgets, work plans, and schedules.
  • Lead CHA program evaluation activities.
  • Meet reporting and financial deadlines.
Communications, Administration, and Funding Support
  • Administer fund applications for partner agencies and grassroots groups.
  • Support service coordination, partner communications, and program website maintenance.
  • Support funding applications.
  • Maintain program files, materials, and electronic records.
  • Perform other duties as assigned.
Required Qualifications
  • Bachelor’s degree in health or social sciences, or an equivalent combination of education and related experience.
  • Experience working with diverse, equity-deserving communities using anti-oppressive, anti-racist, and inclusive approaches.
  • Knowledge of systemic barriers and their impact on access to health, social, and community services.
  • Training or experience in community development, facilitation, group work, program planning, evaluation, adolescent development, or mental health.
  • Demonstrated experience in community outreach, engagement, and relationship-building with clients, partners, and stakeholders.
  • Experience planning, implementing, and evaluating community-based programs while managing competing priorities and deadlines.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Excellent written and verbal English communication skills.
