As a vital member of the Long Term Care team, you'll play a key role in fostering relationships that enrich the daily lives of residents at Glen Rouge Community. This temporary full-time position emphasizes collaboration and relationship-building, ensuring resident satisfaction and well-being through your dedicated care coordination.
Key Responsibilities:
• Oversee resident transitions including move ins and outs
• Work closely with leadership to address family concerns
• Implement resident satisfaction surveys and track feedback
• Coordinate in-service sessions with community speakers
• Assist in developing quality improvement initiatives based on feedback
Requirements:
• Completed College diploma or equivalent healthcare experience
• At least 3 years in related healthcare or social services
• Detail-oriented with a focus on customer service
• Ability to multitask and work collaboratively
• Demonstrated tact and diplomacy in family interactions
