The Community Action Coordinator will work from Access Alliance’s east location - Access Point on Danforth Hub and a new Population Health Clinic located at 44 Mobile Drive.

About the Population Health Clinic

The Population Health Clinic is a neighborhood-based, interdisciplinary primary care model that brings health and community services together in one accessible location. Developed through a partnership of Community Health Centres, including Flemingdon Health Centre, Access Alliance, and South Riverdale Community Health Centre, the clinic is co-located within New Circles, a registered charity that provides free clothing, settlement support, employment training, and community connections to individuals and families facing barriers.

This is designed to create a low-barrier pathway into the health system and build on trusted relationships already established within the community.

Through a collaborative team of physicians, nurse practitioners, registered nurses, allied health professionals, and administrative staff, the clinic will provide integrated, person-centred care that improves access to primary care, advances health equity, and supports better health outcomes for the communities it serves. This is an opportunity to be part of an innovative model of community-connected care that is helping shape the future of primary health care in East Toronto.

A key function of the role is to support the system navigation needs of clients, helping them access and connect with appropriate health, social and community services and resources, while working collaborative with health care providers and community partners to address barriers to care.

Detailed Description Responsibilities:
Community Engagement & Health Promotion
  • Work with marginalized community members including newcomers, immigrants and refugees, volunteers, residents, agency alumni and other organizations to identify health priorities and work in collaboration with the community to plan for and implement strategies to achieve better health. The specific focus of this role is supporting and enhancing the peer outreach women program as well as the community ambassador model.
  • Work with the interdisciplinary team, other organizations, Access Alliance clients and community groups to identify current and emerging needs of immigrants and newcomers; and develop or strengthen programming to address those service gaps. The focus in particular is hard to reach and underserved communities facing barriers accessing social determinants of health, families and youth at risk of violence.
  • Be familiar with and use diverse community engagement and outreach strategies including health education, creating supportive environments, community development, policy building and re-orienting community and supporter services to support people and communities to increase control over and improve their health.
