The Community Action Coordinator will work from Access Alliance’s east location - Access Point on Danforth Hub and a new Population Health Clinic located at 44 Mobile Drive.

Access Alliance Multicultural Health and Community Services (AAMHCS) is a Community Health Centre dedicated to improving health outcomes for the most vulnerable immigrants, refugees, and their communities. By providing direct services and addressing systemic inequities, the Centre envisions a future where Toronto’s diverse communities achieve health with dignity.

The Population Health Clinic is a neighborhood-based, interdisciplinary primary care model that brings health and community services together in one accessible location. Developed through a partnership of Community Health Centres, including Flemingdon Health Centre, Access Alliance, and South Riverdale Community Health Centre, the clinic is co-located within New Circles, a registered charity that provides free clothing, settlement support, employment training, and community connections to individuals and families facing barriers.

This is designed to create a low-barrier pathway into the health system and build on trusted relationships already established within the community.

Through a collaborative team of physicians, nurse practitioners, registered nurses, allied health professionals, and administrative staff, the clinic will provide integrated, person-centred care that improves access to primary care, advances health equity, and supports better health outcomes for the communities it serves. In this role, the Community Health Promoter plans, implements and evaluates community engagement, peer outreach, community ambassadors and related initiatives in accordance with the strategic directions of the organization, and integrates an anti-oppression health promotion focus into the work of the Centre. The community health promoter functions as a member of a multi-disciplinary team working with staff, program participants and the broader community to implement health promotion strategies as outlined in the Ottawa Charter and Declaration of Alma Ata.

A key function of the role is to support the system navigation needs of clients, helping them access and connect with appropriate health, social and community services and resources, while working collaboratively with health care providers and community partners to address barriers to care.

• Work with marginalized community members including newcomers, immigrants and refugees, volunteers, residents, agency alumni and other organizations to identify health priorities and work in collaboration with the community to plan for and implement strategies to achieve better health.
  
  The specific focus of this role is supporting and enhancing the peer outreach women program as well as the community ambassador model.
• Work with the interdisciplinary team, other organizations, Access Alliance clients and community groups to identify current and emerging needs of immigrants and newcomers; and develop or strengthen programming to address those service gaps.
