This part-time temporary position at Humber River Health involves managing a varied caseload related to chronic conditions. Collaborating with an interdisciplinary team, the Registered Dietitian will conduct assessments, implement care plans, and educate patients on nutritional best practices for improved health outcomes.
Key Responsibilities:
• Collaborate with physicians and healthcare team members
• Create and evaluate individual nutritional care plans
• Conduct regular assessments of patient nutritional status
• Develop and deliver nutrition-focused education programs
• Record patient care activities in electronic systems
Requirements:
• Degree in Food and Nutrition from an accredited program
• Membership with the College of Dietitians of Ontario
• Minimum 2 years clinical nutrition experience preferred
• Strong group teaching and adult education skills
• Excellent writing and verbal communication abilities
