Key Responsibilities:

• Conduct psychosocial assessments and crisis interventions

• Facilitate smooth transitions from acute care to community settings

• Provide care coordination and support to clients and families

• Develop relationships with service providers for effective referrals

• Participate in team calls, data collection, and tracking

Requirements:

• MSW or post-graduate degree with 2+ years experience

• Registration with Ontario College of Social Workers

• Minimum 3 years in mental health or addictions

• Strong crisis intervention and communication skills

• Ability to work independently and collaboratively
