The Care Information Manager will oversee the information systems, data workflows, and reporting structures that power the Mino Care Perinatal Pathways Program. This position is essential to maintaining a coordinated, bi-directional data-sharing model that connects hospitals, midwifery clinics, community health centres, and Black-led community organizations.
The ideal candidate understands both health data and health systems navigation, including provincial and federal frameworks governing privacy, interoperability, race-based metrics, and health information exchange.
Key Responsibilities Information System Management- Oversee the development, maintenance, and optimization of care information systems supporting the Perinatal Pathways Program.
- Ensure systems enable secure bi-directional data sharing with all partner institutions.
- Support onboarding and training of staff in documentation processes, data access protocols, and system workflows.
- Ensure adherence to provincial and federal privacy legislation, PHIPA, and applicable data-sharing standards.
- Coordinate the collection, disaggregation, and reporting of race-based and programmatic outcome data.
- Work with partner institutions to align data definitions, reporting cycles, and quality improvement metrics.
- Bridge information needs across hospitals, clinics, social services, and community partners.
- Support the integration of Mino Care staff into partner systems and platforms.
- Collaborate closely with the Care Partnerships Manager to ensure operational and data workflows align.
- Develop data dashboards, summaries, and insights to support monitoring, evaluation, and decision-making.
- Identify gaps in data capture, documentation, or system performance and propose improvements.
- 3+ years of experience in health information management, health informatics, data systems, or related roles.
- Strong understanding of provincial and federal health information systems, privacy legislation, and interoperability frameworks.
- Experience supporting cross-institutional data projects or multi-organization health initiatives.
- Ability to translate complex data concepts for diverse audiences.
- Commitment to culturally safe, equity-focused care and data practices.
