Registered Nurse - NSWOC Nurse (Toronto Central)

RN NSWOC Rate
: $51.94 – $53.66 per hour
Position Overview
This part‑time NSWOC Nurse in the Toronto Central region provides home‑health nursing services, primarily in clients’ homes with occasional clinic visits. A valid G‑class driver’s licence and reliable vehicle are required. The NSWOC program focuses on prevention and reduction of chronic wounds and pressure injuries, delivering specialised holistic assessment and management as a core team member for individuals with ostomies, acute and chronic wounds.
- Assess, monitor, and develop care planning and coordination.
- Evaluate care‑plan interventions through ongoing reassessment and evaluation.
- Ensure quality of care by identifying and addressing factors affecting practice autonomy and delivery.
- Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
- Support safe client care through continuous quality improvement principles.
- Inform nursing supervisor of client activities and concerns; submit appropriate documentation to the case coordinator.
- Maintain documentation standards per the College of Nurses of Ontario.
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- Completion of the Wound, Ostomy and Continence Education Program (WOC‑EP) from NSWOCC.
- Preferred:
Master’s in Advanced Wound Care (MScCH WPC) from UofT, Master of Clinical Science (MCISc) in Wound Healing, or International Interdisciplinary Wound Care Course (IIWCC) from UofT. - Membership of a Professional Association (RNAO) preferred.
- RN diploma/degree from a recognised university or college.
- Valid First Aid or CPR Certification.
Valid G‑class driver’s licence and reliable vehicle. - Experience in Acute Care and Medical‑Surgical nursing – a plus; knowledge of community nursing – a plus.
