Position Summary

University Health Network (UHN) is looking for an experienced Nurse Practitioner to work in the Virtual Behavioural Medicine Program, part of the Geriatric Psychiatry Services at Toronto Rehab. This is a virtual Nurse Practitioner-led service, supporting people with dementia and responsive behaviours, primarily in long-term care homes, across Ontario.

Duties

In this role, the Nurse Practitioner (NP) functions in an expanded, specialized role with a high level of autonomy and expert skill to formulate clinical decisions and support the management of behavioural and psychological symptoms and promote quality of life for people with dementia.

The principal responsibilities include: demonstrating a comprehensive theoretical knowledge base and advanced level of clinical competence in caring for complex patients involving collecting and interpreting data about the health and behaviours of the patient to determine a diagnosis and develop a pharmacological and non-pharmacological care plan; working collaboratively with geriatric psychiatrists and a multidisciplinary mental health team; playing a leadership role and supporting quality improvement and program evaluation;

participating in policy and procedure development; participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies; performing cross-functional and/or other duties consistent with the job classification, with potential career growth and mobility across UHN.

Qualifications
  • Completion of Masters Degree in Nursing required
  • Current College of Nurses of Ontario registration in the Extended Class is required
  • Nurse Practitioner (NP) Adult certificate or Primary Care complete
  • A minimum of 2 years experience working in geriatric care with experience in mental health
  • Expertise and/or certification in management of behavioural and psychological symptoms of dementia
  • Confidence in the care of medically complex older adults with multiple co-morbidities, in particular people living with dementia and residents of long-term care
  • Experience in nursing research, program planning, and evaluation methodologies
  • Comfortable working in a virtual environment
  • Membership in the Registered Nurses Association of Ontario (RNAO): related professional interest group or association preferred
  • Ability and commitment to develop a collaborative practice with medical & allied health teams and in collaboration with Long-Term Care homes
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
