Job DescriptionSinai Health is seeking a Registered Nurse to join the Mount Sinai Academic Family Health Team at the Toronto – Granovsky site. The Registered Nurse provides comprehensive, patient-centred care to assigned patients, with a focus on assessment, health promotion, disease prevention, maintenance, and restoration of health, using the nursing process to deliver high-quality care. In addition to clinical responsibilities, the Registered Nurse contributes to the academic mission of the department by supporting interprofessional education, mentoring learners, and participating in quality improvement and research initiatives
About the Granovsky Gluskin Family Medicine CentreIntegrated with the Department of Family Medicine at Mount Sinai Hospital, our primary care team combines the expertise and services of different health care providers to help patients address many kinds of health care needs. We provide families with comprehensive care at any age. Our services include preventive and acute care, as well as prenatal, birth, postpartum, paediatric, adult, and geriatric care.
We have special programs devoted to diabetes, mental health and perinatal care.

- Provide comprehensive, patient-centred nursing care across the lifespan within an interprofessional Family Health Team
- Conduct holistic assessments, including physical, emotional, spiritual, cultural, and psychosocial aspects of health
- Develop, implement, and evaluate individualized care in collaboration with patients, families, physicians, and interprofessional team members
- Deliver health education, counselling, and coaching to support health promotion, disease prevention, and chronic disease self-management
- Initiate and carry out clinical activities under established medical directives and clinical protocols, including immunizations
- Perform triage to assess patient needs, determine urgency, and facilitate timely access to care
- Coordinate continuity of care by managing referrals, booking investigations, follow-up, and connecting patients to community and health system resources
- Accurately document clinical assessments, interventions, and outcomes in the electronic medical record, in accordance with legal, professional, and organizational requirements
- Plan, organize, and prioritize nursing activities in a dynamic primary care environment, responding appropriately to changing patient needs
- Collaborate with physicians and interprofessional team members to develop and maintain clinical workflows, care standards, and team-based practice
- Teach, support, and mentor learners, including nursing students, physician learners, and new staff, fostering a culture of learning and professional development
- Participate in departmental and organizational committees, including unit council, as needed, to support shared governance and professional practice
- Identify, communicate, and contribute to opportunities for process improvement, risk mitigation, and quality enhancement to support safe, high-quality patient care
- Participate in quality improvement, patient safety, and professional practice initiatives
- Perform other duties as assigned, consistent with the Registered Nurse scope of practice
Mandatory
- Current certificate of registration with the College of Nurses of Ontario(CNO)
- Graduate of a CNO-approved bachelor’s degree in nursing program, or equivalent combined education and experience
