The Acute Care Program is seeking a full-time, permanent Nurse Practitioner who will work within the scope of practice of a Registered Nurse (Extended Class) [RN(EC)] in the Virtual Urgent Care Clinic. When not focusing on the Virtual Urgent Care Clinic, the successful candidate will function as part of the interprofessional team in the Bridging Service. The Bridging Service is a drop‑in, low‑barrier, rapid‑access service that provides short‑term care for clients who have recently been discharged from an acute care inpatient unit or diverted/referred from the Emergency Department.
Reporting to the Manager of the Bridging Service, the Nurse Practitioner will provide comprehensive care to clients, including medication management, housing and financial support, psychosocial support, health assessments, follow‑up and treatment, expert consultation, planning, implementation, evaluation of direct care, clinical leadership, autonomous decision‑making and communication of diagnoses, prescribing medications and ordering diagnostic tests. Additionally, the Nurse Practitioner will lead and manage crisis responses, including suicide risk assessments, and demonstrate knowledge of the Mental Health Act.
Qualifications- Registered with the College of Nurses of Ontario as a Registered Nurse in the Extended Class [RN(EC)], with a specialty certificate in either Adult or Primary Care, and possess a current BCLS.
- Preference for candidates holding or pursuing a Master’s in Nursing or a related graduate degree; CNA certification as a Psychiatric Mental Health Nurse is an asset.
- Minimum two years’ experience or demonstrated expertise working with clients with mental health and/or addiction issues.
- Demonstrated ability to work independently and as part of an interprofessional team, establishing and maintaining effective therapeutic relationships with clients with complex needs.
- Excellent communication and interpersonal skills; demonstrated knowledge of mental health and addictions, and a strong understanding of clinical standards related to Nurse Practitioner scope of practice.
- Experience working with clients from diverse ethno‑racial and cultural backgrounds; ability to collaborate with internal and external stakeholders to support excellence in client care, education and research.
- Strong organizational, decision‑making and problem‑solving skills.
- Computer skills, including word processing, electronic communication and workload measurement systems.
- Ability to work flexible hours and willingness to work day and evening shifts.
- Bilingualism (French/English) or proficiency in a second language is considered an asset.
This full‑time, permanent position is a 1.0 commitment, which may be subject to change under the Collective Agreement. Additional hours may be available based on operational needs. The position is part of the ONA Bargaining Unit and is located at 1051 Queen Street West.
