Responsibilities:
Providing expert clinical care to hospice residents; expert clinical advice to healthcare clinicians at hospice and to nurses on the role of pain/symptom management, psychosocial supports & therapeutic palliative care interventions.Participate in resident rounds and case conferences with the care team routinely & as needed.Work collaboratively with palliative care physician team to assess and manage patients related to their medical care, pain/symptom management, psychosocial assessments, counsel caregivers & residents and to lead prevention initiatives.Collaborate and consult with in-house palliative care clinicians but also external care partners including hospitals, palliative care units, hospices, home care and other community agencies and healthcare providers.Provide advice regarding resident care issues and consult with others when needed.Helpidentify the educational care needs of the interdisciplinary care team and facilitate training to support those needs.Remain current with evidence-based hospice palliative care literature, including best practice guidelines.Communicate and consult with caregivers.Consult with staff on best practice protocols in delivery of hospice palliative care.Contribute to the development, implementation and evaluation of care protocols, best practice guidelines, and new care procedures.As a leader of clinical practice, provide input to the development of the budget, strategic plan and business plan.Participate in hospice palliative care research, developing and implementing practice innovations and program planning.Other duties as discussed and assigned.Required
Skills and Abilities:
3+ years of experience working in hospice palliative care.2+ years of experience working in community care.Demonstrated experience with proven team building abilities and experience in advancing the clinical practice of multiple health disciplines.Demonstrated use of theory and evidence to advance clinical practice & outcomes.Experience working in community & equity-oriented models of palliative care.Experience working with structurally vulnerable populations an asset.Required Knowledge and
Experience:
Excellent clinical nursing skills with experience and proficiency in current physical assessment and treatment methods within the scope of practice.Excellent communication skills (written and oral), interpersonal skills, and administrative skills that include computer literacy.Required Professional Designation/Certificate:
Current registration with the College of Nurses of Ontario in the Extended Class without restrictions.Post-graduate certificate and experience in palliative care an asset (e.g., Certificate in Hospice Palliative Care Nursing Canada [CHPCN(C)]).Completion of a recognized Nurse Practitioner program.Kensington Health Centre is an equal opportunity employer and encourages applicants from equity seeking groups.
