Your role will focus on patient assessments and strategic care management.
This nurse practitioner position is centered on the management of cognitive disorders, including dementia and Alzheimer’s disease. You'll not only engage in direct patient care but also contribute to the development of crucial policies and procedures. By collaborating with neurologists and a variety of healthcare professionals, you will help shape comprehensive care pathways and monitor patient progress effectively.
Key Responsibilities:
• Conduct assessments for cognitive and functional health
• Initiate and adjust patient treatment plans
• Work with an interprofessional team to optimize care
• Contribute to the development of risk management policies
• Provide mentoring and support to nursing staff
Requirements:
• Master’s degree with Extended Class Certification
• Three years of Nurse Practitioner experience preferred
• In-depth knowledge of cognitive disorders
• Current B.C.L.S. Certification
• Strong interpersonal and leadership skills
