Candidates will focus on managing chronic conditions, particularly HIV and Hepatitis C, within collaborative healthcare teams. Key Responsibilities:
• Provide patient care on-site with a focus on harm reduction
• Assess, diagnose, and develop treatment plans for patients
• Prescribe and oversee medication regimens
• Educate patients about health prevention and wellness practices
• Coordinate with local health organizations for comprehensive care
Requirements:
• Active CNO license as a Registered Nurse Practitioner
• Master’s degree in Nursing (MN-NP or MScN)
• Preferred experience in harm reduction and chronic condition management
• Strong clinical decision-making capabilities
• Current N95 Mask Fit Test and required immunizations
