Become a home health nurse with VHA Home Health Care, providing vital care to adults throughout the Central region. This part-time role allows you to build connections while managing your schedule.
We're looking for a compassionate RN or RPN to join our team and provide visiting nursing services. You will coordinate care for diverse clients while adhering to the standards and policies set by the College of Nurses of Ontario. Your contributions will be vital to improving client outcomes and fostering independence.
Key Responsibilities:
• Evaluate and adjust individualized care plans for clients
• Monitor client health and report significant changes
• Maintain thorough documentation per regulatory guidelines
• Collaborate with case coordinators on client updates
• Promote quality care initiatives and client safety
Requirements:
• CNO registered nurse or practical nurse certification
• RPN diploma/degree preferred
• Valid First Aid or CPR certification
• Driver’s license and a reliable vehicle required
• Strong verbal and written communication skills
