Palliative Care Manager at Yee Hong

Lead the delivery of compassionate hospice care as the Manager of Residential Hospice at Yee Hong Centre in Scarborough. Drive excellence in patient-family services and team leadership within a culturally inclusive environment.

As a key player in Yee Hong's residential hospice, you'll assist the Executive Director in ensuring adherence to best practices and regulatory standards in patient care.

Your role emphasizes high-quality nursing services while supervising and training staff to provide exceptional care for diverse communities. Collaborate across teams to maintain a safe and effective environment for patients and their families.

Key Responsibilities:

• Coordinate high quality nursing services for patient/family care

• Supervise patient care team and assess contracted service quality

• Develop interdisciplinary care plans for each patient/family

• Recruit and manage staff to meet hospice needs

• Implement and evaluate quality improvement initiatives

Requirements:

• Registered Nurse with Bachelor’s degree in Nursing

• Minimum 4 years of nursing experience in hospice or palliative care

• Must become Hospice Certified within two years

• Proficiency in Cantonese and/or Mandarin preferred

• CPR Certificate and strong leadership abilities
