CMHA Toronto is seeking a Primary Care Nurse Case Manager to deliver integrated health services. This full-time role focuses on improving client access to care while addressing complex health needs related to chronic conditions and mental health.
As part of a passionate team, you will assess client health, provide nursing interventions, and advocate for necessary services. This position emphasizes continuity of care and aims to reduce barriers related to social determinants of health, ensuring that clients receive the support they need for overall wellness.
Key Responsibilities:
• Conduct thorough health assessments for diverse client populations
• Implement nursing interventions and manage chronic conditions
• Collaborate with various healthcare professionals for coordinated care
• Develop and track personalized care plans with clients
• Advocate for client needs and connect to community resources
Requirements:
• Degree in nursing with Ontario College of Nurses registration
• Minimum 2 years nursing experience in community or primary care
• Skilled in conducting health assessments and managing referrals
• Strong teamwork skills within interprofessional environments
• Familiarity with trauma-informed care principles
