Position: Primary Care Nurse Case Manager Role
CMHA Toronto is seeking a Primary Care Nurse Case Manager to deliver integrated health services. This full-time role focuses on improving client access to care while addressing complex health needs related to chronic conditions and mental health.

As part of a passionate team, you will assess client health, provide nursing interventions, and advocate for necessary services. Key Responsibilities:

• Conduct thorough health assessments for diverse client populations

Requirements:

• Degree in nursing with Ontario College of Nurses registration

• Minimum 2 years nursing experience in community or primary care

• Skilled in conducting health assessments and managing referrals

• Strong teamwork skills within interprofessional environments

• Familiarity with trauma-informed care principles
