Palliative Care Registered Practical Nurse

We are seeking full and part-time RPNs to join our specialized community palliative care team. In this role, you will deliver patient-centered care, including symptom management and emotional support, while collaborating closely with health system partners. Key Responsibilities:

• Collaborate with the interdisciplinary team to create care plans

• Participate in clinical huddles and case conferences

• Provide holistic, patient-centered care and education

• Manage symptoms and offer emotional support to patients

• Travel to patients’ residences within the assigned region

Requirements:

• Registered and in good standing with the College of Nurses of Ontario

• Minimum two years of nursing experience, palliative care preferred

• Experience working with palliative care patients is essential

• Valid Ontario driver's license and reliable vehicle for travel

• Current CPR and First Aid Certificate
