Role Description

This is a temporary, on-site Nurse Practitioner role for a facility based in Bathurst, Toronto, ON. The Nurse Practitioner will provide comprehensive healthcare services, including conducting patient assessments, diagnosing and managing acute and chronic conditions, creating personalized treatment plans, and educating patients on health management. Collaboration with interdisciplinary teams to ensure the best possible patient outcomes is a key aspect of this role.

The Nurse Practitioner will also maintain accurate and confidential medical records while adhering to established policies and regulations.

Qualifications
  • Proficient in patient assessment, diagnosis, and treatment planning
  • Strong skills in health education, patient communication, and care coordination
  • Understanding and adherence to healthcare policies, regulations, and ethical guidelines
  • Experience working in diverse healthcare settings, such as home care, long-term care, or hospitals
  • Current Nurse Practitioner certification and licensure
  • Ability to work collaboratively in interdisciplinary healthcare teams
  • Excellent organizational and documentation skills
  • Commitment to compassionate, patient-centered care
  • Clinical experience with accident recovery or private facility care is an asset
  • Advanced knowledge in specific areas such as chronic disease management or geriatrics is beneficial
