Nurse Practitioner for Adult Care at Unity Health

Join Unity Health Toronto as a Nurse Practitioner focused on resident care operations. You'll provide comprehensive health management while collaborating with specialists and leading quality improvement initiatives.

In this role, you'll work within the Houses of Providence team, reporting to the Director of Care. With at least one year of experience in geriatric or long-term care nursing, you’ll diagnose, prescribe, and ensure quality healthcare. Collaborating closely with an inter-professional team allows you to enhance patient outcomes through effective communication and care strategies.

Key Responsibilities:

• Diagnose and prescribe treatment within your scope of practice

• Collaborate with physicians and healthcare teams for comprehensive care

• Analyze trends in resident care for educational programming

• Lead and support Clinical Quality Improvement initiatives

• Ensure compliance with health and safety standards

Requirements:

• Minimum one year in geriatric, rehabilitation, or long-term care

• Current registration with the College of Nurses of Ontario

• Knowledge of Long Term Care and health policies

• Certifications in relevant nursing specialties are preferred

• Ability to be onsite as required
