Nurse Practitioner in Geriatric Settings

This full NP role involves working with an inter-professional Geriatric Assessment Intervention Network team. You will assess, diagnose, and implement care plans while focusing on older adults’ needs. Join as a leader in educating clients and caregivers to ensure optimal health outcomes.

Key Responsibilities:

• Conduct assessments and consultations for seniors

• Follow NP Scope of Practice regulations

• Direct care that emphasizes patient safety

• Facilitate evidence-based plans for older adults

• Lead educational initiatives for caregivers about care plans
Requirements:

• Baccalaureate Degree in Nursing required

• Completion of Ontario Nurse Practitioner Certification

• Experience working with frail seniors (3-5 years)

• Recent NP experience in community healthcare

• Valid driver's license and vehicle for home visits
