As part of a dedicated interprofessional team, you will provide total care for patients in the palliative stage. Your responsibilities will include assessing patient needs, managing symptoms, and implementing care plans that align with Sunnybrook's Mission and Values. Your palliative care experience will be crucial in this capacities.
Key Responsibilities:
• Offer total care for palliative care patients in need
• Work collaboratively within an interprofessional healthcare team
• Utilize assessment skills to evaluate patient conditions
• Uphold the principles of Person Centred Care
• Apply effective pain and symptom management techniques
Requirements:
• Registered with the Ontario College of Nurses
• Current B.C.L.S certification and RPN diploma mandatory
• Experience in hospice palliative care is required
• Familiarity with Canadian palliative care standards preferred
• Strong interpersonal and problem-solving abilities required
