At Baycrest Hospital, older adults receive specialized post-acute inpatient care informed by deep expertise in aging and brain health, delivered with compassion and clinical excellence. The Complex Continuing Care program cares for older adults with multiple chronic health conditions, providing ongoing in-hospital care that aims to improve the quality of life of medically complex patients.

The Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The program helps patients and their families identify strategies that support quality-of-life goals as well as short term and long term care needs.

• Plan for, deliver, and evaluate nursing interventions, including medications, within the RPN’s scope of practice.
• Provide client-centred care within an interdisciplinary care team to meet personal care, activation, recreation, and spiritual needs of clients.
• Collaborate with the Registered Nurse to develop and revise individualized care plans.

• Current registration in good standing from the College of Nurses of Ontario (CNO) is required.
• Current Basic Cardiac Life Support (BCLS) certificate, Level C - Automated External Device (AED) is required.
• Experience caring for geriatric patients with complex medical care needs preferred.
• Psychogeriatric experience, i.e., mental health assessment and intervention and working with responsive behaviours preferred.
• Excellent interpersonal, communication, and problem‑solving skills.
• Basic computer skills – Meditech, Microsoft Word, and Excel preferred.
