Make a difference in community health as a Registered Nurse with VHA Home Health Care's Palliative Program. This full-time position in the Central region emphasizes compassionate care and client engagement.
At VHA, you will coordinate and deliver palliative home nursing services to diverse clients and families. The role involves assessing patient needs and collaborating with multidisciplinary teams to ensure effective pain and symptom management. Key Responsibilities:
• Develop and implement individualized care plans
• Continuously assess effectiveness of nursing interventions
• Support client safety and quality care practices
• Provide relevant documentation and updates to supervisors
• Engage in standby/on-call responsibilities and team discussions
Requirements:
• Registered with the CNO and in good standing
• RN diploma or degree from an accredited institution
• Required First Aid or CPR Certification
• Previous experience in community nursing is an asset
• Must possess a valid Ontario driver’s license and have a dependable vehicle
