Palliative Care Nurse Practitioner

Position: Palliative Care Nurse Practitioner Role
We are looking for a dedicated Nurse Practitioner at Kensington Hospice to provide quality palliative care. This role emphasizes teamwork, pain management, and expert clinical advice in a compassionate setting.
As a key player at Kensington Hospice, you will spearhead patient assessments in collaboration with our palliative care physicians. Your 3+ years of hospice experience will guide your contributions in support of patient care, team education, and the development of evidence-based protocols. You will also engage with vulnerable populations, enhancing community-oriented palliative services.

Key Responsibilities:

• Deliver expert clinical care to hospice residents

• Guide and support palliative care clinicians with symptom management

• Facilitate training for interdisciplinary team members

• Remain updated on best practice guidelines and research

• Help formulate and evaluate care procedures
Requirements:

• Minimum 3 years in hospice palliative care settings

• 2+ years experience in community care

• Strong backgrounds in team collaboration

• Current registration in Extended Class with the College of Nurses of Ontario

• Nurse Practitioner program completion required
