At VHA, we are seeking dedicated Registered Practical Nurses to support our Hospital Partnerships Program in Toronto. This position involves assessing, monitoring, and coordinating care for clients, ensuring safety and quality. Key Responsibilities:

• Assess and monitor client health and care needs

• Coordinate care plans and interventions effectively

• Ensure a high quality of client care delivery

• Maintain documentation standards as per regulations

• Communicate changes in client care to supervisors

Requirements:

• Valid RPN license with the College of Nurses of Ontario

• RPN diploma from a recognized institution

• First Aid and CPR certification required

• Community nursing knowledge is an asset

• Valid Ontario driver’s license and reliable vehicle needed
