Position Summary
The Medly Coordinator supports the delivery of high-quality care for heart failure patients by managing daily Medly alerts in collaboration with the Heart Function Nurse Practitioner and physicians. This role involves monitoring and triaging patient alerts, communicating with patients and families to support proper use of Medly, and providing education on heart failure management and self-care. The incumbent applies clinical knowledge and best practice guidelines to support guideline-directed medical therapy, including medication titration, while ensuring accurate documentation within the EPIC health information system in accordance with regulatory standards.
Working closely with interdisciplinary teams, referring providers, and organizational partners, the Medly Coordinator prioritizes workload effectively, contributes to continuous quality improvement initiatives, and promotes a collaborative and patient-centered care environment.
