Wound Resource Nurse

Job Summary

Are you a passionate and adaptable nurse looking to make a real impact in the community? We’re seeking a dedicated SWAN Nurse to support all of our Community Programs!

• Support community programs in providing wound care and related services.
• Develop and implement wound care management programs in a hospital or community setting.
• Provide visiting nursing services and collaborate with multidisciplinary teams.

• Completed CNA-accredited Skin Wellness Associate Nurse course (SWAN) or International Interprofessional Wound Care Course (IIWCC).
• Registered Practical Nurse who graduated from an accredited School of Nursing.
• Registered as a member in good standing with the College of Nurses of Ontario.
• Valid driver’s license and reliable vehicle.
• Two years of previous experience in the development of wound care management programs in a hospital or community setting.
• Previous visiting nursing experience considered an asset.
• Updated immunization records, clear criminal background check, and vulnerable sector check upon hire.
• Current CPR certification required.
