This permanent full‑time position (salary $45.85–$57.31 per hour, 37.5 hours per week, Monday‑Friday shift) is located in Toronto General Hospital/Princess Margaret Cancer Centre and reports to the Manager.
The inpatient/outpatient Patient Care Coordinator will support the Classical Hematology Program (Thrombosis Clinic, Red Blood Cell Disorders Clinic, General Hematology Clinic) and the Palliative Care Program (inpatient and outpatient services). These programs provide specialized, interdisciplinary care for patients with complex non‑malignant hematologic conditions, cancer‑related pain needs, and end‑of‑life care across oncology diagnoses.
The portfolio includes leadership responsibilities for administrative staff, registered nurses, clinical coordinators, and allied health professionals working collaboratively within an interdisciplinary model.
Duties- Ensure efficient day‑to‑day operations of clinical areas.
- Implement continuous improvement and project management practices to strengthen clinic processes.
- Perform financial and administrative responsibilities to ensure efficient and effective site operation.
- Contribute to staff development by promoting a supportive/collaborative learning environment based on adult learning principles and nursing practice standards for nurses, students, and ancillary staff.
- Perform human resources responsibilities in collaboration with the nurse manager.
- Liaise with members of the multidisciplinary team to ensure high standards of quality and optimal management of patient care outcomes, including data collection and reporting.
- Completion of a BScN or equivalent Bachelor of Nursing degree (Diploma in Nursing required).
- Master’s degree in Nursing preferred.
- Basic Cardiac Life Support (BCL) certification.
- Three (3) years of nursing experience, including supervisory capacity relevant to the area of responsibility.
- Current licence (certificate of competency) from the College of Nurses of Ontario.
- Active membership in a professional nursing association or related interest group.
- Strong leadership and motivational skills.
- Experience with project management and/or Lean process improvement.
- Teamwork skills and ability to collaborate with others in delivering patient care.
- Excellent problem‑solving, decision‑making, and time‑management skills.
