As a Registered Nurse in the transitional care unit, you will provide comprehensive care for patients with diverse medical needs, including Dialysis, Dementia, and Palliation. Working within a multidisciplinary team, you will embody the Patient-Focused Care philosophy. Strong communication skills and problem-solving capabilities are essential to deliver exceptional patient-centered care.
Key Responsibilities:
• Provide total personal and medical care for residents
• Support patients with Dialysis and complex medical conditions
• Collaborate with the multidisciplinary team for optimal care
• Perform physical assessments and airway management tasks
• Communicate effectively with patients and families
Requirements:
• Current Ontario RN Registration
• B.C.L.S. certification required
• Dialysis and Oncology experience preferred
• GPA or PEICES certification considered an asset
• Strong assessment and conflict resolution skills
