Experienced Nurse Practitioner in Palliative Care

Kensington Hospice is hiring an experienced Nurse Practitioner to deliver effective palliative care for our residents. This role requires outstanding collaboration within a dynamic healthcare team focused on patient-centered care.

At Kensington Hospice, you will utilize your advanced training and over 3 years of hospice experience to provide patient-centered support.

Your role demands high-level interactions with clinical teams, pain management consultations, and ongoing education within the palliative care domain. You will work diligently to promote equitable care for structurally vulnerable populations.

Key Responsibilities:

• Offer advanced clinical advice and pain management strategies

• Engage in interdisciplinary rounds and case discussions

• Collaborate with external care partners on patient management

• Conduct and conclude educational sessions for care teams

• Actively contribute to research and innovative practices

Requirements:

• Over 3 years of hospice palliative care experience

• At least 2 years in community care environments

• Strong clinical and administrative skills

• Current College of Nurses of Ontario registration (Extended Class)

• Completion of recognized Nurse Practitioner program
