Cognitive Care Registered Practical Nurse Position

At Baycrest Hospital, your role as a Registered Practical Nurse will involve providing dedicated support to older adults facing diverse cognitive challenges. You will work collaboratively to deliver effective nursing interventions, assessments, and personalized care plans tailored to individual client needs. This position provides an opportunity to contribute meaningfully to geriatric care.
Key Responsibilities:
• Collaborate with the RN to ensure quality care delivery
• Plan and implement nursing interventions and medication management
• Conduct comprehensive assessments for client needs
• Develop and revise individualized care plans
• Support clients' personal and spiritual well-being
Requirements:
• CNO registration in good standing required
• BCLS Level C certification with AED essential
• Prior experience in long-term care settings preferred
• Psychogeriatric experience is a plus
• Strong communication and basic computer skills required
