Key Responsibilities:
• Work under the supervision of the Integrated Care Program Supervisor
• Create personalized patient care plans with the team
• Attend clinical huddles and case discussions with healthcare professionals
• Provide education for patients and their caregivers
• Travel within your region to provide in-home care
Requirements:
• Registration with the College of Nurses of Ontario
• Two years of nursing experience; palliative experience preferred
• Familiarity with care for palliative patients
• Valid driver’s license and willingness to travel
• Current CPR and First Aid Certificate
