Position: Sunnybrook Registered Nurse - CG-TCU
Join the dedicated nursing team at Sunnybrook Health Sciences Centre as a Registered Nurse in CG-TCU. Provide critical acute care while ensuring a positive patient experience in a fast-paced environment.
As a Registered Nurse in the Transitional Care Unit, you'll be instrumental in offering quality nursing care for patients transitioning in the acute care continuum. You'll work alongside RPNs and other healthcare professionals to implement effective strategy for patient care and education while upholding the highest standards of nursing practice.

Key Responsibilities:

• Conduct assessments and identify patient care needs

• Create and implement nursing care plans effectively

• Collaborate with team members to optimize patient outcomes

• Educate patients and families during discharge processes

• Exercise critical thinking to escalate care as needed
Requirements:

• Active registration with the College of Nurses of Ontario

• At least 1 year of relevant acute care nursing experience

• CPR certification by the Heart and Stroke Foundation

• Familiarity with key medical devices and procedures

• Dedication to ethical, compassionate nursing care
