Unity Health in Toronto seeks an experienced Nurse Practitioner specializing in adult and geriatric care. Join our Houses of Providence team to provide vital patient care and support today.
As a Nurse Practitioner at Unity Health, you will work autonomously within defined legislative scopes. Collaborating closely with a team of physicians and specialists, your role involves diagnosing, ordering tests, and delivering therapeutic management for residents. You will enhance communication throughout the care process while ensuring continuous support.
Key Responsibilities:
• Diagnose and interpret tests within collaborative agreements
• Prescribe medications and perform advanced health assessments
• Collaborate with physicians for integrated resident care
• Lead quality committees and continuous quality improvement projects
• Attend safety in-services and ensure compliance with health regulations
Requirements:
• One year of experience in geriatric or long-term care nursing
• Current registration with the College of Nurses of Ontario
• Certifications in gerontology, mental health, or palliative care an asset
• Knowledge of Ontario's Long-Term Care Act
• Ability to handle responsibilities for health and safety compliance
