Nurse Practitioner – RCC IP (Temporary Full Time)

Position SummaryThe Nurse Practitioner (NP) is a Registered Nurse Extended Class with graduate education in nursing, possessing advanced clinical and theoretical knowledge/skill within the clinical specialty. The NP functions in an expanded role with a high level of autonomy to formulate clinical decisions, manage acute and chronic illness, and promote wellness.
As a member of the interprofessional team, the NP will advance best‑practice care in this new program, conducting comprehensive health assessments, diagnosing and providing treatment, developing and implementing care plans in collaboration with physicians and team members, providing education and follow‑up, and may engage in research, evaluation, leadership, and collaborative activities.
Principal Responsibilities- Work collaboratively with physicians, the interprofessional health team, patients, and families to provide advanced nursing practice through physical and psycho‑social assessments, outpatient care plan development, and case management.
- Implement care plans, assess patient outcomes, and coordinate care with other specialists.
- Assume roles within advanced nursing practice domains: clinical practice, research, leadership, collaboration, and consultation.
- Develop effective partnerships with physicians and other interprofessional team members to provide quality care to patients and families.
- Apply comprehensive theoretical knowledge and approved medical directives within scope to assess and manage patients’ medical care, demonstrating exemplary clinical assessment and decision‑making skills.
- Provide leadership in evaluating quality of care and patient outcomes, partnering with physicians and team members.
- Identify quality initiatives that strengthen and develop nursing professional practice.
- Evaluate the care provided to patients and maintain accurate statistics for reporting.
- Registered with the College of Nurses of Ontario in the Registered Nurse Extended Class with a clinical focus in Adult Care, in good standing.
- Minimum of two years of experience in medically complex care and Complex Continuing Care, demonstrating excellence.
- Experience completing comprehensive assessments on complex patients.
- Experience evaluating quality of care and patient outcomes within specialty populations, partnering with physicians and interprofessional team.
- Demonstrated effective leadership skills.
- Strong interpersonal communication skills with ability to establish positive relationships with all staff, clients, families, and community agencies.
- Current Basic Cardiac Life Support (BCLS) certification.
