We are seeking a Registered Nurse to join our dedicated Palliative Program team in Central Ontario.

This role offers a great opportunity for new graduates and experienced nurses to implement their skills in a supportive environment. You'll coordinate nursing services, ensuring quality and client-centered care for individuals needing specialized support at home.

Key Responsibilities:

• Conduct comprehensive assessments to guide care planning

• Monitor client progress and adapt care plans as necessary

• Ensure compliance with nursing documentation standards

• Communicate effectively with clients, families, and health teams

• Participate in standby/on-call rotation for client support
Requirements:

• Registered with the College of Nurses of Ontario (CNO)

• RN diploma or degree from an approved institution

• First Aid and CPR certification required

• Professional experience in acute care or community nursing is advantageous

• A valid Ontario driver’s license and access to a vehicle
