Palliative Care Registered Practical Nurse

We are seeking full and part-time Registered Practical Nurses (RPNs) to join our specialized palliative care team in Etobicoke. This role focuses on supporting patients and their families through a collaborative, community-based care approach. Candidates should possess strong palliative care skills and a passion for enhancing the quality of life for patients at this critical stage.
Key Responsibilities:
• Collaborate on personalized care plans with the interdisciplinary team
• Attend clinical huddles and case conferences regularly
• Provide nursing care in accordance with CNO standards
• Travel between assigned patients' residences or facilities
• Support patients and families throughout their palliative journey
Requirements:
• Registered with the College of Nurses of Ontario
• Minimum two years nursing experience, ideally in palliative care
• Experience with palliative care patients required
• Valid Ontario driver’s license and reliable vehicle
• Current CPR and First Aid certificate
