We are actively seeking a skilled Nurse Practitioner to manage chronic diseases and conduct health assessments in a collaborative care environment. This role involves working alongside healthcare professionals to provide timely medical interventions.

Key Responsibilities:

• Conduct comprehensive assessments and diagnosis for residents

• Manage chronic conditions including hypertension and diabetes

• Perform routine health assessments and wellness checks

• Prescribe appropriate medications as per NP scope

• Lead care conferences and engage with families

Requirements:

• NP designation with the College of Nurses of Ontario

• Preferred experience in geriatrics or related fields

• Current CPR certification is necessary

• Strong clinical judgment and communication abilities

• Understanding of electronic medical records and Ontario regulations
