Key Responsibilities:
• Collaborate to develop personalized care plans with team input
• Participate in case conferences and clinical discussions
• Deliver holistic care and patient education effectively
• Support the emotional and psychological needs of patients
• Travel to various locations to meet patients’ needs
Requirements:
• Active registration with the College of Nurses of Ontario
• Minimum of two years nursing experience; palliative experience preferred
• Proven experience with palliative care patients
• Ontario driver’s license and car for travel
• Current CPR and First Aid Certificate
