Outpatient Registered Nurse Family Health Team

Become part of a dedicated Family Health Team Clinic as an Outpatient Registered Nurse. Contribute to enhancing patient access to healthcare by working with our interdisciplinary team.

In this role, you will assess diverse patient needs and collaborate on care plans. Responsibilities include monitoring health changes, administering prescribed treatments, and driving health education initiatives. Your clinical skills will significantly impact our patients, enabling them to engage more fully in their health decisions and treatments.

Key Responsibilities:

• Conduct thorough assessments to identify needs

• Administer treatments and manage vaccinations

• Record and monitor patient symptoms and conditions

• Partner with teams to implement and evaluate care

• Ensure comprehensive documentation in EMRs

Requirements:

• Ontario College of Nurses registration required

• Must have primary health care experience

• Strong clinical assessment and communication skills

• Current CPR/BLS certification

• Familiarity with EMR systems, especially Telus Practice Solutions
