Join Baycrest Ambulatory Services as a Registered Nurse (RN) in community settings! Focus on geriatric care, chronic disease management, and wellness education for senior clients.
As part of the Interprofessional Primary Care Team, you will provide health navigation and nursing clinics for seniors living independently in the Toronto area. This role emphasizes chronic disease management and mental health support. Your comprehensive assessments and care plans will promote clients' health goals and enhance their quality of life through close collaboration with physicians and community partners.
Key Responsibilities:
• Deliver client-centered health care from admission to discharge
• Conduct health promotion clinics and community visits
• Develop and facilitate health education programs
• Collaborate on chronic disease care plans within teams
• Liaise with community providers for coordinated care
Requirements:
• Registration in good standing from CNO
•
B.Sc. Nursing Degree and BCLS certification
• One year of relevant geriatrics experience
• Proficient in community services knowledge
• Strong interpersonal and problem-solving skills
