Position: Registered Practical Nurse for Home Care
Join the VHA Home Health Care team as a Registered Practical Nurse in the Hospital Partnership Program, covering North York and Scarborough. This part-time role offers valuable nursing experience with flexible shifts.

As an RPN, you will have direct responsibility for patient assessments, care plan development, and quality assurance. You will ensure that nursing practices are in line with professional standards while fostering supportive relationships with clients and their families.

Key Responsibilities:

• Conduct thorough patient assessments and evaluations

• Create and revise care plans based on patient needs

• Uphold nursing documentation standards to ensure quality

• Communicate effectively with clients and nursing supervisors

• Engage in continuous quality improvement for patient care

Requirements:

• Registered with the College of Nurses of Ontario

• RPN diploma or related degree required

• Valid First Aid or CPR certification

• Familiarity with community nursing is an asset

• Valid Ontario driver’s license and reliable vehicle for travel
